What are Social Determinants?
Social Determinants of Health are all the factors outside of medical care that can affect health outcomes, such as individual behaviors, socio, economic and physical circumstances. Examples include where we live and work, housing, age, genetics, physical activity, income level, diet, education, and access to care. According to a recent American Hospital Association report, 80% of health outcomes are driven by social determinants. This means healthcare and staying healthy begin long before we go to the doctor.

What is the Healthcare Problem?
Healthcare costs continue to skyrocket while health outcomes stagnate. Patients are treated under the assumption that their medical history is all that’s needed to make care decisions. Why is this? Healthcare leaders have recognized that SDoH have a dramatic negative impact on the cost and delivery of healthcare in America, but don’t know how to gather SDoH data at scale. Payers and Providers struggle to identify the individualized patient's social determinants, which help complete the picture of the patient, and then act on them to improve patient outcomes and decrease costs. Currently, both payers and providers are attempting to address socioeconomic factors at a community level instead of at an individual level. As a result, most payers and providers are missing 60%-80% of the factors contributing to an individual’s health

Bellrock enables health insurers and health systems to solve this problem of operationalizing social determinants in three distinct ways:
Social determinants are scattered across thousands of external data sources and are not easily obtained. The data tends to be messy, incomplete, and unstructured. Further, much of the data is only available at the block group or zip code level. Bellrock provides SDoH data specifically tied to individuals, enabling patient/member-centric predictive modeling. For example, we provide air quality data at the patient’s specific address, which can help inform the patient and medical personnel about heightened risk for respiratory disease and actions to mitigate the risk.
Once social determinants are obtained, fusing them with existing data such as claims and/or medical records is the next step. This fusion allows for integration into existing workflows, which helps care managers understand a patient’s story before even meeting them. For example, the care manager can view the patient’s medical history, while also grasping their SDoH constraints. This can better inform the care manager to treat the clinical and non-clinical risks.
After obtaining and merging SDoH with claims and/or medical records, the next step is to take action. Bellrock can, if desired, create individual risk scores and predictive models to identify high-risk individuals (“Bloomers”), and educate staff on how to maximize the value of the newly created models.
